to use your results to create a wellness plan.

What is SHALA?
SHALA (Stanford Health and Lifestyle Assessment) is a confidential online health survey and takes about 20 minutes to complete. Your answers provide the basis for the five reports you will receive. The SHALA reports:
- Help you link your current lifestyle behaviors to your risk for certain chronic diseases.
- Outline your risk for type II diabetes, cardiovascular disease and other health issues.
- Provide you with health behavior targets to reduce your risk and improve your health.
- Give you strategies to reach your health goals.
How do I access SHALA?

What happens to the information I report on SHALA?
All information is considered confidential and will be stored on the secure and confidential Stanford School of Medicine servers that follow all the University security guidelines.
Can my manager, supervisor, administrator or health plan see my SHALA responses?
