14 or higher after a blood draw no warfarin and taken 15 mg of Vitamin K and had 3 units of FFP what are possible causes

I've been on warfarin for 2 years for PE two months after having liver and kidney failure caused by a combination of dehydration from the "Norwalk" flu virus. I have a family history of clotting (Father-multiple strokes/heart attacks and Mother had a filter put in a return artery from multiple PE's) I have stopped taking the warfarin for 5 days, have had 3 units of fresh frozen plasma blood products and 15mg of vitamin K and still have my labs come back at 14 or higher (whatever that means). What could be the possible causes of not clotting?
