What is the safest way to wean off of Plavix? I've been on it for 5 years.

I had a brain stem ischemic stroke 5 yrs ago, have used Plavix since.  Also have become very athletic, lost all the extra weight, went on a plant-based, whole grain, no oil eating plan.  I want to get off the Plavix.  The 90-day rebound report scares me.  How do I wean off safely?  Thanks.
