Fistulas on buttocks cheeks

I am 24. wheelchair parson.I hace fistulas on both buttocks.They are opened by a doctor in Poland and he said it should close themseklves.Now it is a year after I went out of hospital (spend there 6 months) and it's still the same opened and maybe only a little bit smaller.Please tel me if You can help me??I have Visa to USA and I can come any time....Just help me.Money is not a problem.Please answer.
