distended upper abdomen

I am 41 years old and look 8 months pregnant - definitely not pregnant though - I have lots of belly fat which my doctor says I need to reduce - but my upper abdomen is so distended - starting right under my rib cage -

I am 5 feet 6.5 inches tall and weight 200 lbs.  The rest of my body doesnt' look overweight - just my belly - but several people have asked me if I am pregnant-  what reason could my upper abdomen be distended?

I asked my doctor the same thing and she told me the possibility of fat under my organs and muscles. A healthier diet seems to be a long term solution.
