Most women have some difference in nipple position between breasts, but with implants a larger difference can represent capsular contracture or internal breast scarring. Without knowing whether or not implants are present or where she stood before implant surgery, it is impossible to know what is going on here, but there is enough here to fix. I'd wager that she has implants.
